Friday, July 10 – 4:30pm

Registration:  3:15pm – 4:15pm at Arnie’s Ford Mercury (3rd St. Location)

 

Register in the following ways:

  1. Register the day of the Contest between 3:00pm & 4:30pm

 

REGISTRATION INFORMATION:

1.  Name of chicklette(s)

2.  Age of chicklette(s)

3.  Birthdate

4.  Name of parents

5.  Phone # parents can be reached at

6.  (optional)  Please put anything that your

chicklette likes to do or any special talents they might have!

Age Groups:

Group #1:  Ages 0 – 12 months

Group #2:  13 months – 24 months              

Group #3:  25 months – 4 year old

*Prizes will be given for first place and runner up in each category*

 

Contest will start at 4:30pm so everyone can get to the festivities that start at 6pm!

Please feel free to dress your ‘chicklette’ up in if you would like!  There are no rules, except that it is mandatory that everyone come to have fun!  (parents can come up with the children also)
